The Outreach program is a program that assists with many needs to keep older persons independent. Efforts are made to identify and contact older persons who are isolated and in social or economic need and assist them in receiving appropriate services.
For example, the Center was able to provide a Hillsdale County senior a ride home from the hospital after being discharged because she didn’t have any family in town.
